The treatment itself will involve a combination of various treatment methods, therapy and possible medication. We will only recommend medication if necessary. Forms/Letters Policy:

If you have not seen the Physician in over a year, please call the office for an appointment before your request will be reviewed.

Form requests are not accepted at the first appointment visit.

Allow 3 – 5 business days for processing.

There is a fee for filling forms & writing letters which is due once the forms/letters are ready and prior to receipt. The fee $25 (Letters) & $50 (Forms) and applies to each form request.
